All attendees, members or guests, are required to complete this registration form if they are attending our Sunday Service in person.

This allows us to contact attendees in the rare or unexpected event that there is a positive COVID case at our event.

In consideration of the opportunity to participate in the activities of the Delaware Church [herein listed as “THE CHURCH”, I/we release THE CHURCH, its officers, agents, employees, staff, and volunteers from any and all liability of any kind whatsoever for any loss or injury to me/us arising from my/our participation in the activities of THE CHURCH; and I/we agree to indemnify and hold forever harmless the THE CHURCH, its officers, agents, employees, staff, and volunteers from any and all liability of any kind whatsoever for loss or injury to me/us arising from activities on or off the premises of THE CHURCH or resulting from traveling to or from the activities of THE CHURCH, including loss or injury resulting from negligence or gross negligence. I/we understand and agree that this permission and agreement shall remain in effect until revoked in writing by me/us.
